Few think sluggish U.S. economy will strengthen soon

WASHINGTON (AP) ? A U.S. economy that plodded along in the first three months of the year likely grew even less in the April-June quarter. And most economists no longer think growth will strengthen much in the second half of 2012.

Weaker hiring, nervous consumers, sluggish manufacturing and the overhang of Europe's debt crisis might be pointing toward everyone's big fear: another recession.

Against that background, the government on Friday will issue its first of three estimates of how much the U.S. economy expanded last quarter. The consensus forecast is that growth slowed to an annual rate of 1.5 percent, according to a survey of economists by data firm FactSet. The Commerce Department will issue the estimate at 8:30 a.m. EDT.

A quarterly growth rate of 1.5 percent would be the weakest in a year. It would follow a meager 1.9 percent rate in the first three months of 2012.

Much more growth would be needed to fuel stronger hiring. Economists generally say even 2 percent annual growth would add only about 90,000 jobs a month. That's too few to drive down the unemployment rate, which is stuck at 8.2 percent.

The U.S. economy has never been so sluggish this long into a recovery. The Great Recession officially ended in June 2009.

Until a few weeks ago, many economists had been predicting that growth would accelerate in the final six months of the year. They pointed to gains in manufacturing, home and auto sales and lower gas prices.

But threats to the U.S. economy have left consumers ? who account for about 70 percent of the economy ? too anxious to spend freely. Jobs are tight. Pay isn't keeping up with inflation. Retail sales fell in June for a third straight month. Manufacturing has weakened in most areas of the country.

Fear is also growing that the economy will fall off a "fiscal cliff" at year's end. That's when tax increases and deep spending cuts will take effect unless Congress reaches a budget agreement.

All that is making companies reluctant to expand and hire much.

From April through June, U.S. job growth slowed to 75,000 a month, down from a healthy 226,000 average in the first three months of the year.

"The European situation has been getting worse and is dragging down the global economy," said Sung Won Sohn, an economics professor at California State University. "And we have got the fiscal cliff to worry about in the United States."

Six of the 17 countries that use the euro currency are in recession. Growth has also weakened in powerhouse emerging markets in China, India and Brazil. With these economies slowing, so is their demand for U.S. exports.

Sohn estimates the likelihood of a U.S. recession within the next 12 months at 30 percent to 35 percent. That's up from his estimate of 20 percent six months ago.

Nariman Behravesh, chief economist at IHS Global Insight, puts the chance of a recession at 25 percent. He expects growth to increase slightly to an annual rate above 2 percent in the second half of this year.

Other economists are gloomier. They think growth will muddle along below 2 percent through 2012.

Many economists think consumers pulled back sharply on spending last quarter. Analysts at JPMorgan estimate that consumer spending grew at a scant 1 percent annual pace in the April-June period, down from a 2.5 percent annual increase in the first quarter.

"Businesses and consumers are quite worried, so they're holding back," Behravesh said. "For consumers, the worry is the jobs markets. Businesses are worried about Europe. And China is looking weaker than most of us would have thought even a few months ago."

Behravesh said even companies that think Congress will manage to reach a budget deal by year's end are too uncertain about possible tax changes to step up hiring.

"That is making them very cautious about investment decisions," he said.

In delivering the Federal Reserve's midyear economic report to Congress last week, Chairman Ben Bernanke sketched a bleak picture. And he warned that unless lawmakers strike a deal, the tax increases and deep spending cuts that will take effect Jan. 1 could trigger another U.S. recession.

Bernanke has said the Fed is prepared to take further action if unemployment stays high. He hasn't specified what steps it might take or whether any action is imminent.

The lackluster economy is also raising pressure on President Barack Obama in his re-election fight with Mitt Romney, the presumptive Republican presidential nominee.

But few think the Fed, the White House or Congress can or will do anything soon that might rejuvenate the economy quickly. Many lawmakers, for example, refuse to increase federal spending in light of historically large budget deficits.

"There is nothing out there to light a fire under the economy," said Joel Naroff, chief economist at Naroff Economic Advisors.

Summary

Grady Emergency Medical Services collaborated with the Emory University Department of Emergency Medicine to develop an out-of-hospital cardiac arrest registry in the city of Atlanta. Through this initiative, known as the Cardiac Arrest Registry to Enhance Survival program, Grady identified weaknesses in its dispatching process and made changes to decrease delays in handling 911 calls. Evaluations suggest that these improvements, in conjunction with a community-wide training program on cardiopulmonary resuscitation, led to a significant increase in the cardiac arrest survival rate in Atlanta.

Evidence Rating

Moderate: The evidence consists of pre- and post-implementation comparisons of dispatch time, response time, and survival rates. Out-of-hospital cardiac arrest is the leading cause of death among adults in the United States, killing approximately 325,000 individuals each year, the majority of whom die before reaching the hospital. The Greater Atlanta area experiences many deaths due to out-of-hospital cardiac arrest each year, deaths that could potentially be prevented by the rapid arrival of emergency medical services (EMS) personnel.

  • Many preventable deaths nationwide: Only approximately 20 percent of cardiac arrest victims who are found in a shockable rhythm (patients with the greatest likelihood of survival) currently survive the event. Out-of-hospital cardiac arrests can be treated successfully through early intervention, including cardiopulmonary resuscitation (CPR), defibrillation, and advanced cardiac life support, but only if this treatment is delivered within minutes of collapse. Each minute of delay reduces the chance of survival by roughly 10 percent.1 Up to 50,000 deaths could be prevented through use of CPR and an automated external defibrillator (AED) at the time of the emergency.2
  • Large variations across communities: Nationally, few communities actively monitor and report their survival rates from out-of-hospital cardiac arrest. Among those that do, survival rates vary more than 10-fold, from 2 to 35 percent.3 Low rates can be attributed to both poor professional and community-level response to out-of-hospital cardiac arrests. Fragmented interagency communications often result in delayed EMS response, whereas a lack of community awareness frequently leads to low rates of bystander intervention.1
  • Atlanta area a poor performer: Between December 2006 and June 2007, Atlanta's overall survival rate for cardiac arrest averaged 1.8 percent, while the Utstein rate (which considers only those patients most likely to benefit from resuscitation) was 4.2 percent. Delays in EMS arrival contributed to these low rates, as few patients were found in a shockable rhythm when emergency responders arrived.1

Description of the Innovative Activity

Grady EMS used data from the Cardiac Arrest Registry to Enhance Survival (CARES) program to identify and address weaknesses in the EMS communication system and to streamline its 911 dispatching and ambulance deployment processes. The system is used on an ongoing basis to monitor performance and promote continuous improvement.

  • The CARES database: The CARES database includes the following information:
  • Response times: Grady EMS reports and tracks its EMS response times, including receipt of the 911 call, dispatch of the ambulance, and on-scene arrival of the ambulance.
  • EMS data: Grady EMS uses CARES to collect data related to the treatment of cardiac arrest, including location of the event, bystander intervention (if any), etiology of the arrest, first arrest rhythm, and destination hospital.
  • Hospital outcomes: Area hospitals report discharge information for cardiac arrest patients transported to their facility, including outcomes in the emergency department and hospital and neurologic status at the time of discharge.
  • Initial identification of communication and deployment problems: Analysis of CARES data from December 2006 to June 2007 (before implementation of the program) revealed significant delays in ambulance dispatching. At that time, the established process required three separate steps before an ambulance could be dispatched to the scene of an emergency: 911 calls first came to the Atlanta police department, which determined the nature of the emergency; the call was then transferred to a Fulton County dispatcher who established the priority of the medical emergency; and then the call was transferred to Grady EMS to dispatch an ambulance. Not surprisingly, the use of multiple transfers resulted in significant time delays; on average, it took 1 minute, 47 seconds from receipt of the 911 call to dispatch of an ambulance and more than 7.5 minutes for an ambulance to reach the patient.
  • Communications consolidation and streamlining: In June 2007, Grady EMS consolidated its communications system and removed Fulton County from the dispatching process. Calls came into Atlanta's police department and then transferred directly to Grady EMS.
  • Strategic deployment of ambulances: To facilitate faster response times, Grady EMS began deploying ambulances throughout the city, often sitting in shopping center parking lots or other busy areas. Grady EMS chose locations based on CARES data that identified high-risk areas for cardiac arrests.
  • Ongoing monitoring and continuous improvement potential: A steering committee that includes representatives from each hospital and agency meet regularly to review performance and determine additional streamlining and changes that could improve EMS response times and patient survival. Improvements are not limited to reducing ambulance response times. For example, some departments have expressed interest in identifying new communication technologies to allow paramedics to communicate a patient?s condition more effectively to hospital staff. However, budget cuts made to the county's EMS budget during the 2008 to 2009 fiscal year have prevented acquisition and development of new technologies.

Did It Work?

A comparison of CARES data during the 6-month period before and after implementation found that the changes made by Grady EMS reduced the average ambulance dispatch time and led to a significant improvement in survival rates among out-of-hospital cardiac arrest victims in the Atlanta area.

  • Reduced dispatch time: The average time between the receipt of a 911 call and the deployment of an EMS ambulance fell by more than 1 minute, from 107 seconds to 43 seconds. The average time from the 911 call to ambulance arrival fell by 32 seconds, from 7 minutes, 37 seconds to 7 minutes, 5 seconds after implementation of the improved response system. However, budget cuts made during the 2008 to 2009 fiscal year reduced the county's EMS operating budget by nearly one-third. As a result, there are fewer ambulances available on standby to respond to calls, and response times have begun to increase, although specific response data is not available.
  • Improved survival rates: Overall survival rates among out-of-hospital cardiac arrest victims increased from 1.8 to 4.2 percent, and the total number of cardiac arrest survivors increased from 2 during the 6-month period before implementation to 5 during the 6 months after implementation.4 The Utstein* survival rate rose from 12.5 percent before implementation to 14.7 percent during the first year and to 22.2 percent during the second year of implementation, between October 2006 and September 2007. However, because budget cuts have reduced the number of ambulances available to respond to calls, the Utstein survival has declined to 5.7 percent between October 2007 to September 2008.

*The Utstein rate is an internationally accepted standard for measuring out-of-hospital cardiac arrest outcomes for patients most likely to benefit from resuscitation.

Context of the Innovation

Grady Health System's EMS, the designated 911 ambulance provider for Atlanta, is the largest hospital-based ambulance service in the nation, serving 142 square miles. Grady EMS employs 280 paramedics, responds to 96,000 calls, and transports approximately 67,000 patients annually. At peak times, Grady EMS has as many as 28 ambulances on the road. Grady EMS' participation in CARES was implemented at approximately the same time as two other major initiatives in the Atlanta area?Mayor Shirley Franklin's mandate that all 8,000 city employees be trained in CPR, and The American Heart Association's Restart Atlanta's Hearts program to teach 50,000 Atlanta residents how to perform CPR and apply a publicly accessible AED. Planning and Development Process

Key planning and development steps related to participating in CARES include the following:

  • Choosing a method to collect EMS data: Data for the CARES registry can be collected in one of three ways: (1) completion of an optically scannable form, (2) direct entry into the Web-based database, or (3) automatic data extraction from an electronic patient care report.
  • Assigning an EMS agency contact: CARES requires that a program contact be identified at each participating EMS agency; this individual serves as the local site administrator for the agency, oversees CARES operations, and serves as the liaison between the EMS agency and CARES staff.
  • Encouraging hospital participation: CARES encourages voluntary participation from hospitals that receive cardiac arrest patients from participating EMS agencies. Participating hospitals also assign a CARES contact person who receives notification via an automatically generated e-mail to provide outcomes data on cardiac arrest patients. The hospital contact enters data directly into the registry via the CARES Web site. The hospital data set consists of five simple questions, including emergency department outcome, hypothermia treatment, hospital outcome, disposition location, and neurologic status at time of discharge. Although not required, representatives of a participating hospital can sign a CARES data use agreement acknowledging the confidentiality of data exchange.
  • Collecting computer-aided dispatch data: The data are collected through automatic export/import and/or direct entry into the Web-based database by the computer-aided dispatch system administrators who have also signed participation agreements. Administrators enter three data elements (time of call, ambulance dispatch, and ambulance arrival) for each event, with events being matched based on date, approximate time, and location.

Resources Used and Skills Needed

  • Staffing: This program relies primarily on existing staff. As noted, a CARES contact person must be identified to oversee the data collection process at each participating agency.
  • Costs: There is no fee to participate in CARES. The only cost relates to staff time spent on the program. The startup phase requires the most staff time, including planning, training, and implementation. Once operational, the program requires little staff time for ongoing maintenance.

Funding Sources

The Centers for Disease Control and Prevention (CDC) has increased its funding to the program to approximately $500,000/year. The funds cover the continued expansion of the CARES Registry into many new communities across the country and the hiring of an additional coordinator to coordinate its implementation. Originally, the CDC provided $1.5 million over 5 years (2003 to 2008) for Emory University's Department of Emergency Medicine to develop the CARES Registry. Grady funded the changes in 911 call routing.

Getting Started with This Innovation

  • Develop an implementation plan: This step should include a timeline for becoming operational and a data collection plan.

  • Reach out to key stakeholders: Approach hospitals to elicit their participation and encourage local political, business, and community leaders to actively and visibly endorse the program.

  • Continually monitor performance: Evaluate data generated by CARES on an ongoing basis to identify opportunities to improve cardiac arrest survival in the community, including improvements in how 911 calls are dispatched, the placement and deployment of ambulances, EMS response times, the placement of AEDs, and bystander interventions.

Use By Other Organizations

As of April 2012, CARES has now expanded to more than 50 sites across the country. In addition, the group is working at the state-level with eight states and internationally with Singapore and multiple Pan-Asian countries as collaborative participants.

Aston Villa edges Timbers on penalties

PORTLAND, Oregon (AP) -Aston Villa defeated the Portland Timbers in a penalty shootout after a 2-2 draw in Tuesday's friendly.

After both teams successfully converted their first four spot kicks, Aston Villa goalkeeper Brad Guzan stopped Bright Dike's penalty.

That set up a chance to win for Samir Carruthers, who put his shot into the top left corner.

The game had ended square after the Timbers came alive in the final 15 minutes.

Aston Villa controlled the first half, getting a goal in the 41st minute off a header from Ciaran Clark and having another disallowed when Stephen Ireland was called offside.

Portland's Sebastian Rincon scored off a corner from Franck Songo'o to tie the score at 1-1 in the 76th minute.

Just a few minutes later, Matthew Lowton scored for Aston Villa by heading in a corner.

However, Rodney Wallace drilled a left-footed goal from the top of the box in the 81st minute to tie it up again for Portland and send the game to penalties.

The Timbers have been a dramatically different team at raucous Jeld-Wen Field, where they are 5-2-3 this season, than on the road, where they are 0-2-8.

Aston Villa was completing a three-game preseason tour of America, having defeated the Philadelphia Union 1-0 on July 18 and the Chicago Fire 1-0 on Saturday.

Aston Villa will look to rebound from a disappointing 16th-place Premier League finish last year when it kicks off the season Aug. 18 against West Ham.

What Fraction of Bankruptcies Is Caused By Medical Debt?

We don?t have to look very far for an answer. It seems that the authors are reluctant to take their respondents seriously when it comes to an understanding of their indebtedness. The survey clearly states that only 29 percent of the respondents believed that their bankruptcy was actually caused by medical bills. This number sounds like a reasonable approximation of the actual impact of medical debt on bankruptcies. In fact, it accords well with other findings in the literature, including my own. In an AEI working paper, I showed that when medical debts (as a fraction of income) rise by 10 percent, we are likely to observe an increase in bankruptcy filings induced by those debts by approximately 27 percent. In other words, if between 2001 and 2007, medical debts increased by about 10 percent (as a fraction of income), the study suggests that about 27 percent of the total filings during this period can be attributed to medical debts. The SCF shows that for all families, total debts as a fraction of income actually have increased by roughly that 10 percent figure over this period. Therefore, even if all of this increase was due to medical debts (which is unlikely since mortgage debts are ten times higher), the additional bankruptcy filings due to medical debts should be 27 percent of the total. Another paper by Domowitz and Sartain estimates the fraction of medical bankruptcies at about 30 percent, half of what the Warren study finds.

Stocks drop for a 3rd day as earnings slide

NEW YORK (AP) ? A parade of grim news, from weak corporate earnings to a pullback at U.S. factories to spreading fault lines in Europe's debt crisis, sent investors fleeing stocks for a third straight day on Tuesday.

As if that weren't bad enough, Apple delivered a rare earnings disappointment after the closing bell, boding poorly for Wednesday's trading.

The Dow Jones industrial average fell 104.14 points, or 0.8 percent, to 12,617.32. It was the third triple-digit point loss in a row for the blue chip index. The last time that happened was September, when fears were rife that the U.S. was on the brink of another recession.

Lower earnings forecasts from corporate bellwethers like United Parcel Service, combined with a weak report on manufacturing, fed fears of more disappointing results from Corporate America in the coming days.

"Our guess is we haven't seen the worst," said Carl Yingst, chief market analyst at Joseph Gunner, an investment bank.

Soon after he spoke came a bit of confirmation from a stock market star. After the close of trading, Apple reported the smallest increases in revenue and income in years, badly missing analysts' expectations. The stock fell $29.76, or 5 percent, to $571.19 in extended trading.

"It's a huge swing and a miss for a company that usually knocks the cover off the ball," said Jack Ablin, chief investment officer of Harris Private Bank. "The ill winds of global trade are enveloping everyone, even the high and mighty."

It was a fitting end to a bad day as investors around the world dumped stocks and fled to the relative safety of U.S. government debt. The yield on the benchmark 10-year Treasury note fell to another record low and the dollar hit a two-year high against the euro.

Stocks fell from the start of trading following news that UPS had cut its earnings forecast 4 percent for all of 2012 as global trade slows. UPS's stock fell $3.61, or 5 percent, to $74.34.

Also weighing on stocks, Spain's borrowing costs spiked as investors worried that country could become the latest in Europe to ask for a financial lifeline. Spain's banks have already received help from international lenders.

The broader Standard & Poor's 500 fell 12.21 points to 1,338.31. The Nasdaq composite was off 27.16 points to 2,862.99.

Early in the day, DuPont reported that its net income fell 3 percent for the second quarter on slower business in Europe and Asia. The huge chemical maker also reported revenue that fell short of Wall Street's expectations. DuPont's stock lost 97 cents, or 2 percent, to $47.74.

Adding to the jitters was a report from Federal Reserve Bank of Richmond indicated that manufacturing in the central-Atlantic region is contracting. That followed reports of pullbacks in New York and Philadelphia.

In Europe, the yield on the 10-year Spanish government bond rose 0.10 percentage point to 7.53 percent, a dangerously high level. Investors also sold Italian government bonds, sending the yield on that country's 10-year bond up 0.32 percentage points to 6.54 percent.

Late Monday, Moody's Investors Service issued a warning about the credit rating for Germany. Moody's anticipates that strong countries like Germany will have to shoulder a heavy financial burden as they support weaker countries like Spain and Italy. The debts of those countries are considered far too big for current bailout funds to handle.

In cutting its outlook on Germany, Moody's also said there was an "increased likelihood" that Greece would leave Europe's monetary union.

"Things are only going to get worse," said Adrian Day, president of Adrian Day Investment Management. He added that he's not buying stocks, save for gold-related companies, because he expects them head lower as the European crisis deepens.

As they dump stocks, investors have been piling into U.S. government bonds. On Tuesday, the yield on the 10-year Treasury note fell to 1.40 percent, matching the record low it reached Monday.

Investors have also been selling the euro. The euro fell to $1.20 on Tuesday, a two-year low against the dollar.

In corporate news, AT&T and Whirlpool both fell short of analyst estimates of earnings and revenue. AT&T fell 75 cents, or 2 percent, to $34.63. Whirlpool, the world's biggest appliance maker, dropped $5.06, or 7.5 percent, to $62.25.

So far this earnings season, nearly seven of ten companies that have reported earnings have beaten estimates, slightly higher than is usual, according to S&P Capital IQ, a research firm. But analysts had been lowering those expectations for months so the feat of surpassing them isn't so impressive.

Some other companies in the news:

? Cisco Systems fell the most of the 30 stocks in the Dow, or 6 percent, after the network equipment maker announced its latest round of staff cuts. Cisco dropped 95 cents to $15.12.

? DeVry plunged $6.76, or 25 percent, to $20.80, the biggest drop in the S&P 500 index. The for-profit education company said enrollment is falling, forcing it to cut 570 jobs. The company also projected that its profit for the latest quarter will come in well short of analyst estimates.

This boat was found grounded in Lake Washington in Kirkland Friday night and King County Marine Unit was searching Saturday for the registered owner, Richard Sweezey, of Mercer Island.

A search on Lake Washington was under way this weekend for a missing Mercer Island man, after his boat was found running ashore in Kirkland, unoccupied, on Friday night.

The King County Sheriff's Office began an investigation on Friday night when a Kirkland resident along Lake Avenue West called police about a boat drifting to the shore.

After inspecting the boat, deputies found the boat is owned by Richard Sweezey, who lives on Mercer Island.?Other paperwork indicated the boat is moored at a marina in Portage Bay, where?deputies?found Sweezey's vehicle, but not Sweezey.

King County Sheriff Officers, the U.S. Coast Guard and Seattle Police Marine Patrol have searched Lake Washington in the area between the marina and where the boat was found in Kirkland, but did not find any signs of Sweezey.

Anyone who may have seen the boat or who has information is asked to contact the Sheriff's Department at (206) 296-3311.

Jensen, Jensen, Reagan and Robinson continue Keuka College Golf Classic dominance

For the fifth time in six years, the team of Kyle Reagan, Roger Jensen, Scott Jensen and Josh Robinson won the Keuka College Golf Classic.

The team returned a 17-under 55 to capture the 13th annual scramble event held July 16 at Lakeside Country Club in Penn Yan.

The foursome of Mike and Jenn Sweet, Mike Heib and Bryan Lanahan captured the mixed team division, while the senior title was claimed by Dana Schillinger, Gordon Bascom, Ed Perry and Andy Bascom.

The tournament, which drew 124 golfers, benefits the Deb Manahan Golf Classic Scholarship, named for the late Penn Yan resident, avid golfer and Keuka College benefactor. In its first 12 years, the tournament raised some $149,000 for the scholarship, awarded annually to Keuka students from the Finger Lakes region.

(AP) ? From dark days to a critical turning point in the AIDS epidemic: The landscape has changed dramatically in the two decades since the world's largest AIDS conference last met in the United States.

Back in 1990, the first good medicines were still a few years away. Before they arrived, caring for patients with HIV was like "putting Band-Aids on hemorrhages," said the leading U.S. AIDS researcher, Dr. Anthony Fauci.

Fast forward: Today's anti-AIDS drugs work so well they not only give people with HIV a near-normal life expectancy, they offer a double whammy ? making those patients less likely to infect other people.

Sunday, the International AIDS Conference opened in the nation's capital with the goal of "turning the tide" on HIV. Even without a vaccine or a cure, the goal is to finally stem spread of the virus, using that so-called "treatment as prevention" and some other powerful protections.

"There is no excuse, scientifically, to say we cannot do it," Fauci, infectious disease chief at the National Institutes of Health, told reporters Sunday.

But the challenge that more than 20,000 scientists, doctors, people living with HIV and policy-makers will grapple with this week is how to get to what the Obama administration calls an AIDS-free generation. Where's the money? What works best in different countries and cultures?

And with HIV increasingly an epidemic of the poor and the marginalized, will countries find the will to invest in the most vulnerable?

"Rich countries think, 'Who cares? We have the treatment, I hear now that HIV is a chronic condition,'" a worried French Nobel laureate Francoise Barre-Sinoussi, co-discoverer of HIV, told The Associated Press. "I really think there is not the same political commitment as it was in the past."

That political commitment must expand to fight laws that are driving some of the populations most at risk ? gay and bisexual men, sex workers and injecting drug users ? away from programs that could help protect them from getting or spreading HIV, said Michel Sidibe, director of UNAIDS, the United Nations AIDS program.

"It's outrageous that in 2012, when we have everything to beat this epidemic, that we still have to fight prejudice, stigma, exclusion," he said.

People living with HIV marched through downtown Washington Sunday to urge the public and policy-makers to pay attention to a disease that, in this country, doesn't get much publicity anymore.

"We're everyday people. Anybody and everybody can catch this," said Ann Dixon, who traveled from Arkansas to attend the march. She learned she had HIV in 1997.

The AIDS conference ? remarkable for giving a forum not just to leading scientists but to everyday people who live with HIV ? hasn't returned to the U.S. since 1990, in protest of the longtime ban on people with the virus entering the country. The Obama administration lifted the travel ban in 2010, finishing a process begun under the Bush administration. Not lifted was a ban on sex workers and injecting drug users, and protesters briefly interrupted the opening news conference to decry their absence from the meeting.

The conference comes at a time when scientists increasingly say they have powerful new tools to add to tried-and-true condoms.

Studies show treatment-as-prevention, treating HIV right away rather than after someone is sick, lowers patients' chances of spreading the virus through sex by a stunning 96 percent. Already, Fauci said regions that are pushing to get more people tested and rushed into treatment are starting to see infections drop, from San Francisco and Washington to part of South Africa.

The U.S. Food and Drug Administration just approved use of a daily AIDS medicine, Truvada, for use by healthy people hoping to lower the risk of infection by a sexual partner. Hard-hit poor countries are grappling with how to get that protection to their highest-risk populations.

Other goals include getting more HIV-infected pregnant women treated to protect their babies, and getting more men circumcised in developing countries to protect them from heterosexual infection.

But the hurdles are huge.

Since the first reports of AIDS surfaced 31 years ago, a staggering 30 million people have died from the virus and 34.2 million now are living with HIV around the world.

There's still no cure and no vaccine. For every person who starts treatment, two more are becoming infected.

In poor countries, a record 8 million people are getting HIV drugs, but the United Nations says it will take up to $24 billion a year ? $7 billion more than is being spent now ? to reach those most in need.

The epidemic is worst in developing countries, especially in Africa. Progress has stalled even in the U.S., which has seen about 50,000 new infections every year for a decade. Here, nearly 1.2 million people live with HIV, and one in five don't know it.

In Turkey, the land of kebabs and sweet lokum, expanding waistlines are the target of a new anti-obesity campaign by the government to help one million Turks slim down over the next year.

The numbers are staggering: a little over one out of every three people is obese, according to health ministry figures. Even more when it comes to women.

?The fight against obesity starts now,? say publicity spots rolled out by the ministry to push back against lifestyle changes doctors believe are bulking up the 73 million population.

?Modern-day life has set constraints that make us eat faster and more without paying attention to the quality of the food we?re ingesting,? said paediatrician Murat Tuncer, a specialist in blood disorders.

But on the upside, he added, as a Mediterranean country Turkey has all the vegetables, fruit and fish required for a healthy diet.

The ministry sounded the alarm on the problem last month.

?Thirty-five percent of the population is obese,? said Health Minister Recep Akdag, who himself recently set an example by losing 10 kilograms (22 pounds) and recommends a walking regime of 10,000 steps a day.
